Applicants are required to complete the forms and supplementary material as outlined on the Ontario Universities’ Application Centre website and our Application Process page. The following forms can also be used as needed by the appropriate applicant types/individuals. All forms on this page are subject to change without notice.

*To be completed only by those applicants who wish a third party to speak to the University regarding any matters pertaining to their application for admission to an undergraduate program.

This permission implies access by the third party to all of your personal information including grades obtained or reported on the documents submitted in support of your application for admission.

This release does not extend to any matters external to the undergraduate admissions process nor is it specific to any other University department’s transactions. It is highly recommended that applicants give serious consideration to allowing a third party to act on your behalf before completing and submitting this form.
